Abstract
Introduction: Clostridioides difficile (CD) is the most common cause of healthcare-associated diarrhea. The use of antibiotics is considered one of the risk factors for the development of clostridial colitis. The emergence and spread of CD strains that are resistant to multiple antimicrobial agents complicates the possibilities of prevention and treatment of infections caused by CD strains.


Methods: The identification of CD was confirmed by the MALDI TOF mass spectrometry method. Disk diffusion test was used to determine the susceptibility of CD strains to 6 antimicrobial agents (metronidazole (MTZ), vancomycin (VA), tigecycline (TGC), doxycycline (DO), teicoplanin (TEC), rifampicin (RD). The automatic analyzer of inhibition zones BACMED 6iG2 was used to determine antimicrobial susceptibility of CD strains.
Results: The analyzed set consisted of 100 clinical CD strains that were isolated from stool samples of patients with suspected clostridial infection. Resistance to DO and TEC was confirmed in one strain. Two strains were resistant to MTZ and four strains were resistant to VA. Resistance to RD was confirmed in 68 strains.
Conclusion: Antimicrobial resistance of CD plays important role in managing the epidemiology of clostridial colitis. Currently, CD strains with reduced susceptibility to MTZ and VAN as first-line AL used in the treatment of colitis are of particular concern. In clinical practice, it is important that susceptibility testing of clinical strains of CD to AL becomes part of routine laboratory diagnostics (Tab. 1, Fig. 5, Ref. 20).
