SUMMARY
Background: Occurence of extraskeletal myxoid chondrosarcoma (EMC) in the genitourinary tract is extremly rare. This case report presents diagnostics, treatment and follow-up of a patient with an extremly large EMC of the scrotum.
Case report: A 58-year-old male without relevant comorbidities presented with a non tender huge scrotal mass. The mass was first noted about 15 years prior to consultation, with rapid growth in the last 6 months. The major complaint of the patient was his restricted mobility due to the scrotal mass (size 23x17x14 cm, weight 3500 g). Imaging investigations showed a solid mass in the paratesticular space, normal testicles and multiple pulmonary metastases. The patient underwent bilateral orchidectomy through inguinal incisions followed by removal of the entire scrotum. Operation wound healed without complications. As there is limited experience with any other treatment options except surgery, we were reluctant to use them. Despite progression of pulmonary metastases, the patient’s quality of life increased and remained stable during first 12 months after surgery. Subsequently, he suffered from several attacks of bronchopneumonia and died 36 months after surgery.
Conclusions: EMC represents an extraordinary tumour in the scrotum. Its behaviour appears to be that of a slow-growing malignancy. An incomplete tumour resection might not have resulted in overall survival benefit but it improved the patient’s quality of life.
