Summary
Background: The aim of the study was to detect influence of the growing adherence to basis therapy on key clinical and functional parameters in the patients with COPD.

Patients and methods: The 156 patients with severe and very severe COPD were enrolled. The study consists of 5 visits during 12 months. During each visit the necessity to take basis therapy regularly was closely discussed with the patients, the patients demonstrated their ability to use inhaler correctly and initial training/ retraining for its usage was conducted to them. At each visit mMRC and CAT were completed, spirometry was performed and data regarding COPD exacerbations was collected.
Results: The adherence to basis therapy increased from 44.41 ± 1.07 % to 75.21 ± 0.78 %. During 12 months of the patients‘ participation in the study CAT diminished from 29.12 ± 0.43 points to 24.99 ± 0.33, i.e. on 4.13 points. The mMRC grade decreased from 2.14 ± 0.05 to 1.85 ± 0.04. The FEV1 increased on 20 ml (1.16%), FVC – on 90 ml (2.22%), FEV6 – on 50 ml (1.96 %) and FEF25 – on 60 ml/s (0.91 %). The total quantity of COPD exacerbations decreased on 58.0 % and the quantity of severe COPD exacerbations decreased on 56.4 %.
Conclusion: During the patients‘ participation in the study their adherence to COPD basis therapy taking increased from 44.41 ± 1.07 % to 75.21 ± 0.78 %.
