Súhrn
Autor sa v článku zaoberá problematikou využitia techník regionálnej anestézie a analgézie a neuromodulačných (NM) postupov v liečbe tak bežných, ako aj závažných stavov chronickej bolesti. Po úvodnej historickej poznámke a náčrte patofyziologických účinkov nervových blokád sa venuje deleniu anestéziologických techník z hľadiska rozsahu a lokalizácie ich aplikácie, podľa účelu ich aplikácie a dĺžky a spôsobu podávania lieku. Spomína tiež používané farmaká a využívanie fyzikálnych vplyvov pri anestéziologických a neurolytických technikách.
V časti článku o NM technikách definuje ich podstatu, uvádza kritériá výberu vhodného pacienta a pracoviska, ako aj ich indikácie podľa typu a charakteru bolesti. Delí ich na elektrostimulačné a farmakologické s priblížením mechanizmu patofyziologického účinku jednak podávaných farmák, jednak neurostimulačného efektu elektrického prúdu. V časti o farmakologickej NM uvádza indikácie ich použitia, ale aj podrobnejšie rozoberá katetrizačné techniky a implantabilné pumpové systémy. V časti o stimulácii nervových tkanív ich delí podľa miesta pôsobenia elektrického prúdu na mozgovú, spinálnu a periférnu neurostimuláciu s predstavením základných techník stimulácie, predovšetkým implantabilných spinálnych stimulátorov a transkutánnych techník periférnej neuromodulácie (Rebox, TENS). Uvádza tiež ďalšie neuromodulané techniky ako je rádiofrekvenčná termolýza a kryoterapia.
V závere zdôrazňuje význam postavenia anesteziologických a neuromodulačných techník ako súčasti komplexnej liečby závažných stavov chronickej bolesti s nutnosťou ich používania skúsenými odborníkmi.

SUMMARY
In his paper the author deals with the problems of utilizaton of techniques of regional anesthesia and analgesia and neuromodulatiom (NM) procedures in the therapy of both common and serious chronic pain. After the introductory historical comment and outline of patho-physiological effects of the nerve blokades the author classifies the anesthesiologic techniques from the aspect of the scope and localization of their application, according to the purpose of their application and according to the duration and mode of drug administration. He lists commonly used pharmaceuticals and the use of physical effects in anesthesiologic and neurolytic techniques.
In the part on neuromodulation techniques the author defines their nature, presents the criteria of the choice of a suitable patient and workplace and their indications according to the type and character of pain. He divides them into electrostimulation and pharmacologic and elucidates the mechanism of the pathophysiologic effects of the pharmaceuticals administered as well as the neurostimulaton effect of the electric current. In pharmacologic neuromodulation procedures the author presents the indication of their application and analyzes in detail catheterization techniques and implantable pump systems. In the part about the stimulation of nervous fibers the author gives their division according to the site of electric current effect upon cerebral, spinal and peripheral neurostimulation. Simultaneously, the author presents basic techniques of stimulation, such as implantable spinal stimulators and transcutaneous techniques of peripheral neuromodulation (Rebox, TENS) and other neuromodulation techniques, such as radiofrequency thermolysis and cryotherapy.
In the conclusion the author pinpoints the significance of the role of anesthesiologic and neuromodulation techniques as a part of complex therapy of serious states of chronic pain and their use by well-experienced experts.
