SUMMARY
Obesity with its negative health impacts results in the decreased quality of life and shortened life expectancy, mainly via induction of cardiovascular alterations, type 2 diabetes mellitus, oncologic diseases and osteoarthritis. However, obesity and concomitant comorbidities can be manifested at different qualitative and quantitative levels. Recently, a so called obesity paradox was described, when persons had a lower risk of cardiometabolic abnormalities. Definition (not exactly delineated) describes normal lipid and glycid parameters and normotension.
Lower amount of ectopic fat (visceral fat including that in the liver), increased fat deposition in extremities, expansion of subcutaneous fat, preserved insulin sensitivity and pancreatic beta-cells function and better cardiorespiratory fitness are also manifested. Despite the presence of some positive parameters, the risk remains still higher by these patients compared with healthy lean persons. It seems, that this fenotype has transient characteristics, therefore it cannot be considered quite a safe condition, and requires certain therapeutical interventions. Risk stratification of these particular persons should be based on the principle of personalised medicine.
