Abstrakt
Aminotransferázy ALT a AST patria medzi diagnosticky najdôležitejšie enzýmy. AST je enzým s ubikvitárnou distribúciou, kým ALT je enzým relatívne špecifický pre diagnostiku ochorení pečene. V práci píšeme o jednotlivých indikáciách vyšetrenia aktivity aminotransferáz. Hlavnou indikáciou vyšetrenia aktivity aminotransferáz je podozrenie na ochorenie pečene.
Postupne rozoberáme zmeny aktivity aminotransferáz pri jednotlivých typoch poškodenia pečene – akútne a chronické hepatitídy, alkoholové poškodenie pečene, liekové poškodenia pečene, cirhóza pečene, hepatocelulárny karcinóm, ochorenia pečene spojené s graviditou a metabolické ochorenia pečene. Okrem ochorení pečene sú indikáciou pre vyšetrenie aktivity aminotransferáz aj ochorenia kostrového svalstva a myokardu. V závere článku sa v krátkosti zaoberáme problematikou makro-AST a jej významu v diferenciálnej diagnostike stavov so zvýšenými aktivitami AST (tab. 3, obr. 1, lit. 49). Alanine aminotransferase and aspartate aminotransferase and their significance in laboratory diagnostics
Abstract
Aminotransferases ALT and AST are among the most important diagnostic enzymes. AST is an enzyme with a ubiquitous distribution, while ALT is a relatively specific enzyme for the diagnosis of liver diseases. In the article, we discuss individual indications for determining the activity of aminotransferases. The main indication for determination of the activity of aminotransferases is the suspicion of liver disease. We gradually discuss changes in aminotransferase activities in individual types of liver damage – acute and chronic hepatitis, alcoholic liver damage, drug-induced liver damage, liver cirrhosis, hepatocellular carcinoma, liver diseases associated with pregnancy and metabolic liver conditions. In addition to liver diseases, diseases of the skeletal muscles and myocardium are also indications for the determination of aminotransferase activity. At the end of the article, we briefly discuss the problem of macro-AST and its importance in the differential diagnosis of conditions with increased AST activities (Tab. 3, Fig. 1, Ref. 49).
