Rare disease in rare condition: chronic thromboembolic pulmonary hypertension after splenectomy (case report)

SUMMARY
Chronic thromboembolic pulmonary hypertension is a rare severe disease significantly reducing quality and length of life. The accepted hypothesis states  that it is the result of the organization of thromboembolic material and vascular remodeling. The most important clinical risk facor for disease is overcome and reccurent pulmonary embolism and/or deep venous thrombosis. One of the independent clinical risk factors is rare clinical condition - splenectomy.

The disease is currently treatable by surgical - pulmonary endarterectomy. For non-operable patients, specific drug and interventional treatment is available, which have managed to reduce the otherwise high morbidity and mortality of the disease in recent years. To achieve this and make medical management successful, 2 key steps are needed: physicians´ knowledge of this nosological unit, but especially urgent and high-quality diagnostics, the essential part of which is echocardiography and ventilation-perfusion lung scintigraphy.
Key words: chronic thromboembolic pulmonary hypertension, splenectomy, ventilation-perfusion lung scintigraphy.
