Summary
The liver is an organ that significantly interferes with the metabolism of lipids and lipoproteins. It is the site of synthesis of apolipoproteins, VLDL, LDL as well as cholesterol and phospholipids. Chylomicrons and LDL remnants are catabolized in the liver. Thus, it is understandable that impaired liver function will also be accompanied by disorders in the plasma lipoprotein spectrum. Changes in plasma lipid spectrum are an important risk factor for the development of cardiovascular diseases. The aim of our study was to assess changes in the spectrum of lipoproteins in patients with fatty liver disease.

The studied group consisted of 64 patients with hepatic steatosis (32 men and 32 women). The control group consisted of 26 clinically healthy individuals. In each patient we examined the basic lipid parameters - total cholesterol, LDL-cholesterol, HDL-cholesterol, non-HDL cholesterol, triacylglycerols and apoproteins AI and B. We also calculated the lipoprotein ratios - total cholesterol / HDL-C, LDL-C / HDL- C, nonHDL / HDL-C, apoB / apoAI, LDL-C / apoB and atherogenic plasma index (log TAG / HDL-C). The examined lipid parameters showed changes in the group of patients with fatty liver disease compared to the control group. The levels of total cholesterol, LDL-C, nonHDL cholesterol and triacylglycerols were increased, while the levels of HDL-cholesterol tended to decrease. When evaluating the frequency of pathological values of all lipid parameters and lipoprotein ratios, it was found that 91% of patients with fatty liver disease had at least one lipid parameter pathologically elevated. Examination of the LDL-C / apoB index did not show a tendency to increase the proportion of small dense LDL particles. Based on atherogenic plasma index, cardiovascular risk was increased in more than 50% of patients with fatty liver disease. The value of atherogenic plasma index was positively correlated with the severity of hepatic steatosis expressed by the fatty liver index. Examination of lipid parameters and lipoprotein ratios showed changes in plasma lipid profile and an increased risk of cardiovascular disease in a significant proportion of patients with fatty liver disease. It is therefore important to keep this in mind when caring for these patients.
