Abstract
Background: Full-thickness reconstruction of large alar defects requires complex surgery, often in several time-consuming stages. This study presents a modified alar reconstruction technique using a septal graft, skin graft, and nasolabial flap that can be used to repair large alar defects relatively simply.

Methods: This prospective study included sixteen patients who, between January 1, 2020 and December 31, 2022, underwent reconstruction after malignant tumor excision in a modified manner. This differs from standard methods in that we achieve a satisfactory aesthetic and functional result in patients who refuse staged reconstruction after a single procedure.
Results: The cohort included patients aged 60 to 92 years (mean 76.6 years), with a slight preponderance of women (56 %) compared to men (44 %). The causes of defects were excisions of malignant tumors with a prevalence of basal cell (81.2 %), basosquamous (12.5 %) and squamous cell carcinoma (6.3 %). Postoperatively, we observed common complications, which did not determine the success of the reconstruction. We did not observe nostril collapse, hypertrophic scars, or necrosis of flaps or grafts. Overall, the method brought excellent functional and aesthetic results with minimal patient discomfort.
Conclusion: Compared with the existing reconstructive methods of large full-thickness alar defects, the modified septal graft technique can achieve an acceptable aesthetic and functional result in a one-stage procedure (tab. 1, obr. 2, lit. 14). Text v PDF www.lekarsky.herba.sk.
